RAS request form
Use this form to request a restricted access switch (RAS). The form must be submitted by
one of the individuals legally authorized to bind the customer/community who owns
and maintains electric gates.

An authorized representative from the customer's gate company should use this form
to request reimbursement. The form is used to identify the location of the RAS(s) so they can be inspected prior to reimbursement.
