Account information release form
Arizona law now requires SRP to have written authorization from our customers to share
account information with a third party, such as other utility companies, courts or energy
service providers. Previously, customers were allowed to give verbal consent for SRP to
release information.
For your convenience, we are offering an online consent form that you can print out and fax,
mail or deliver to SRP to allow us to release your account information. Because we need to
have your signature on file authorizing us to release your information to a third party, we
cannot allow online completion of this form. This is for your protection.
